Major IRCC Medical Exam Changes – Effective August 21, 2025.

Starting August 21, 2025, Immigration, Refugees and Citizenship Canada (IRCC) has introduced important updates to medical exam requirements for certain applicants, including those applying under Express Entry and some in-Canada immigration programs. Upfront Medical Exam Now Mandatory for Express Entry Applicants

If you’re applying under Express Entry, you now must complete your Immigration Medical Exam (IME) before submitting your application.

This is known as an upfront medical exam. You must include proof of your completed exam with your application.

What this means for you: Start planning early to ensure your medical exam results are ready before you submit your electronic application for permanent residence (eAPR).

Planning Ahead Is Key

With the new mandatory requirement, last-minute preparation could cost you valuable time or even result in a refusal. To avoid issues:

  • Book early with an IRCC-approved panel physician.
  • Keep all documentation from your exam, including the receipt and IMM form provided by the doctor.
  • Upload proof along with your application to avoid processing delays.

Extended Medical Exam Policy for In-Canada Applicants

For applicants already inside Canada, there’s some good news. IRCC has extended its IME policy until October 5, 2029. If you completed an Immigration Medical Exam within the last five years, you may not need to undergo a new one. Simply include your IME number or unique medical identifier from your previous exam in your application.

This extension can save time and money for eligible applicants, but you’ll need to confirm your eligibility before skipping the exam.
